Who create, update the form or table and when

Hi,


I have a form where I need to have who create, update the form or table and
when.
Something likes this, but I do not know how to use this code. Is there
somebody who can take me trough whole procedure how to create such a fields?

CreatedAt [TimeStamp ]
CreatedBy [LAN UserID]
UpdatedAt
UpdatedBy

I am using the Local Area Network when everybody has the unique user name.

I was looking everywhere go get some information how to create these fields,
but I cant find anything.
