Why is information missing when I send my custom form?

I have created a custom contact form in Outlook 2003. I have saved and
published it to our public folder. We need to fill it out and send it to
other co-workers for their viewing. When it sends--some of the filled in
information disappears. Mainly check box selections. Any suggestions on
why?

Sounds like you missed one of these steps:

1) Create Yes/No custom properties to hold the values that the users will enter in the check boxes

2) Add controls to the form bound to those properties.

3) When sending a contact item, include it as an attachment to an RTF-format message.
