Why loosing custom keyboard shortcuts

I have set several custom keyboard shortcuts for my macros in
Normal.dot in Word 2003.

Some of them later cease to work and no longer appearing in the
'Current keys' window in the 'Customize Keyboard' dialog box.

Others still work even though they are no longer present in the
'Customeize Keyboard' 'Current keys' window.

What is happening?
