Word 2007-Inserting pictures with captions

Do you have to insert a frame to be able to apply captions to a picture? If
so, is the correct order to insert the frame first and will the style of the
paragraph apply to the picture as well, such as, 12 pt space after?

then why, when I insert a picture without a frame, does the "insert caption"
option not appear? When would you want to use a frame?
