Word document with two or three columns

I do not know how to make a document for Process recordings. I need space at
the top of the first page for a brief description, underneath two and/or a
third column for recording infomation in each. The next page contining the
bottom format. Help>

I'm not familiar with the task you need to perform, so it's a bit difficult to
judge what kinds of columns you need: newspaper ("snaking text") or a table?
Which version of Word do you have? And what is "bottom format" (how should it
appear on the page)?
