First, while you are still mad, write a letter to scansoft or the maker of
this. It is their fault. Don't send it, just write it.

Then, having vented, ...
Tools > Customize

When the dialog pops up, drag each of those OmniPage commands from your menu
into the middle of your document and let them disappear.

Close the customize dialog box.

Tools > Options > Save
Check the option to prompt you before saving changes to normal.dot.
I know that you don't have this checked because if you did you would have
written here before because this prompt would have tipped you off that
something was wrong, or at least annoyed you.

Close the Options dialog by clicking on OK.

Close Word. When asked if you want to save changes to normal.dot, answer
Yes.

Reopen Word. Are the commands all gone or is there a new one there? If gone,
you are done. If not, you still have an Add-In that you need to eliminate.

When your problem is fixed, add up the time you've spent trying to fix this
problem (that never would have been a problem if the folks at OmniPage knew
what they were doing with Word) and bill them for it with your letter. You
can add in $50.00 for my time. If you collect, give my $50.00 to your local
humane society.

Hope this helps.
