I have seen it in several pages created by frontpage in the <img> tag. As in:

<a href="firetruck.jpg">
<img src=firetruck_thumb.jpg" xthumbnail-orig-image="firetruck.jpg"
width="400" height="300"></a>

Is this in a photo gallery?
If so, it is created by the script that generates the gallery.
