"You Received a Phone Call" Form - How do you make a shortcut?

In the Choose Forms button, the "You Received a Phone Call" form is an option
to select. We use this form for phone messages. How do you make a shortcut
for this form and be able to place it in the Shortcut folder of Outlook?
