endnotes for each chapter moved to end of book

i have just written a book using WORD 2004 with nine chapters and put
the endnotes at the back of each chapter...now i need to place them all
at the end of the total document still arranged by chapters and
numbered to beging with 1 for the NOTES section of each chapter. the
publisher wants me to submit the entire manuscript as one document
instead of separate chapters. i cannot figure out how to get all the
endnotes at the back and keep the numbering to match each chapter
starting with 1 for the notes section for each chapter.
any suggestion or help? the publishing house has no IT person either
who knows how to do this.

You need a section break between chapters (Insert | Break | Section Break
(Next Page).

Then you need to set the endnotes to restart numbering at each section
(Insert | Footnote, Options, then self-explanatory--click OK to exit without
inserting a new note, or cmd-z will undo the note insertion but not the
change in setting).

At the same place, set the endnotes to appear at the end of the document
instead of the end of the section. However, *if* you need a bibliography
after the endnotes, see #6 on this page instead:
