Accepted updates not reaching Project Professional schedule

We have a situation where we obtain weekly updates via PWA from several team
members. Our Project Manager uses the accept all option, then selects
update. This action spawn MS Project Professional but all updates are not
recorded within the project schedule. This happens with updates from team
members within one schedule, and between multiple schedules - we just never
know what updates will be recorded. Is there a step we are missing, or a
best practice available to prevent this rework? After we find the issue, if
we have the team menber adjust their %-complete entry slightly and
retransmit, our acceptance seems to work.
