Adding a check box to a subform

I am building a db to try to alias diferent customer name variations. I have
created a query where I am matching the first 10 characters on a customer
name and it bring back a list of different matches. My idea is to build a
subform where the user could pick the correct alias by clicking on a check
box and after that to create a query to bring back only the records that were
selected. I am not sure how to add the check box to each one of the listed
items in the subform. I will really appreciate your help.

Different idea ---

Bring back a list of different matches in a multiselect listbox using the
first query as the rowsource. Then use the values selected in the
multiselect listbox as criteria in the second query.
