Appreciation card for the work place

I am looking for an appreciation card or note template that I can print and
make copies but I have to be able to fold it so it looks like a card and in
the inside it has lines where you write what that employee did that they
deserve the appreciation note. It says presented to: from: and the
reason why with lines. Can anyone help me? Do anyone know what I am talking
about?

Yes I did, but it didn't see what I was looking for. I was wondering was
their any more a person can choose from?

Templates can be used as a skeleton. I doubt very much if you will find
exactly what you are looking for. This is an ideal time for you to use your
creative spirit. Start deleting what's already here and have fun with the
task at hand.
