Approved Button on Word Document

I need to add a button to an existing Word document that will place the
user's name and the current date and timestamp, to be used as an electronic
signature. I know there are lots of holes in security using this approach but
for simplicity and time sake, I believe this approach will work. I have
never added a button to a document before, does it matter if it is a document
or form? Can I retrieve the user's name from the Options - User Information
window? I need to keep it simple...and quick!
