Arrange by category seems not to work reliably

I use categories to tag emails for when I need to deal with them
(1.Urgent, 2.Next Few Days, etc.) I also have categories of: Personal,
Office, Needs KB article, etc. I then have a custom view that is set to
arrange by category with grouping enabled. Although this works most of
the time, it seems that sometimes the categories that emails are under
do not actually match the categories assigned to them.

The only way I can fix this is to choose to arrange by something else
and then go back to sort by categories. In addition sometimes an email
will have the same category applied multiple times, which makes things
somewhat difficult from a sorting perspective as well. Does anyone else
see these issues?

I haven't seen this behaviour, but I don't group by category on a regular
basis. Could it be that those items with more than one category are sorted
differently, because of the order in which those categories are applied -
e.g is categoru {urgent, personal} the same as category {personal, urgent}?

I have once seen items get the same category multiple times. I have no idea
how it got in that state, and was only resolved by setting the category to
'none' then re-applying the correct categories.
