Attachments as icon in email

Attchments which I send appear as an icon within my message area. They used
to appear in an attachment bar.
What have I changed and how do I reset it

Where the attachment is placed is dependant on the message format, Rich Text
and the attachment is within the body, Plain and HTML in its own field.
