First, check if your host already has a solution.
or Find what technologies your host supports - PHP, Perl/CGI, ASP etc.
Then look Google for form handlers in the appropriate technology.

you need a cgi script - there are thousands of resources on the internet for
these.
try www.hotscripts.com for starters.
