Changing outlook default contact form for exports from ACT

Hi, I want to be able to export my act database to outlook 2003. I have
created a form in outlook and I was wondering how I can make it the default
form so I can export from ACT version 6.0 and have all my fields filled. Any
help would be great. Thanks Sameer
