Creating a Form Using Option Buttons

I'm attempting to create a survey form which uses Option Buttons on a
line-by-line basis. I've tried creating a table for the entire form, and
creating a table for each line. However, it only allows me to choose one
option for the entire form.

I'd appreciate anyone's help!
