Creating a (kind of) questionaire?

I am creating an interactive troubleshooting presentation and was wondering
if you guys could help?

Customers will have a number of specfic symptoms(5-10) of a problem with a
system. What I want to do is; list the symptoms, you select the symptoms you
have, search and a list of possibles appears.

What is the best way to do this and to have it used in Powerpoint? I have a
vision of a check-the-box and search button (???).

Any comments much appreciated...

Yes, it has to be in powerpoint. I read, however, that I could create this as
a macro in excel and then import it into PPT. Any thoughts?
