Cross-Functional FlowChart

Visio Professional 2003 (11.3216.8028)
Operating System: Windows XP Professional

Menu: File > New > Business Process > Cross Functional Flowchart (US Units)
Pop-up asking for number of bands, band orientation, etc is NOT displayed.
I get a blank form without bands, heading, etc.

Second characteristic: Open existing Cross-functional Flowchart. Select
and drag 3rd band up between first and second band; let go(release mouse
button); 3rd band overlays portion of first and second band.... first and
second band do not separate and third band becomes second band.

Removed Visio: Menu: Start > Control Panel > Add / Remove Programs

Reloaded Visio

No Solution: Same characteristics as before.

P.S. This is a recent problem... all worked normally 6 months ago... have
not loaded any new software.

What is causing this?
