Custom Questionaire by email

Hi All

I am trying to create a new custom questionaire with Checkboxes and drop
down fields that I can email to clients for responses.
I can create an email with checkboxes (using the Web tools toolbar) and send
it and the recipient can see the checkboxes, however when they click on
reply, the boxes disappear and they can answer the questions.
Is there a way to create such a questionaire that when they click reply,
they are able to answer the questionaires (checkbox or drop down) and send it
back?
Iam also having trouble trying to work out how to populate the drop down
boxes with the list of potential answers!
