Database Results

I'm having problems setting up the Database Results. I've
gone through the Wizard, which is fine. But, when the
search form displays, part of the coding also shows.

Please check out http://www.aaw-fc.co.uk/test.asp to see
what I mean.

Any help would be greatly appreciated. Thanks.
