How do I create a new design for slide layout as a template, so I can apply
it to the slide when I need it?

You can't add or modify the slide layouts that come with the product, but you
aren't confined to them. You can choose any layout for a new slide and modify
and/or add to it to meet your needs. Then duplicate that slide whenever you
want that particular layout.
