Designing a form for viewing in e-mail

In the Word "Help" There is a section called Overview of forms. On this help screen there is a selection for E-mail forms and it gives the following help: Use e-mail forms to distribute an electronic version of your form, etc. It also shows an example with field labels and fill in fields. When I look for additional help on this topic there is none. So my question is how do I create an e-mail form with locked field labels and fill-in fields? I know how to create a form in Word and send it along as an attachment from Outlook, but what I want is the form to be displayed in Outlook when the receiver opens the email

For what you want to do, Word is simply the wrong tool. Outlook has a form
designer of its own. For a tutorial on it, see
