disappearing custom menu-item

In Word 2003, I created a macro (with a shortcut key), added it to the Tools
menu, and exited Word. When I reinvoked Word, the new menu-item had
disappeared. The macro itself remains present in Normal.dot, and the
shortcut-key still works. But I can’t add any menu-items to the menu-bar.
How can this problem be fixed?

Might this be related to Acrobat 7?

Jean-Guy, thanks. But no, I've got Adobe Reader 9.1.0. And there's no Adobe
toolbar on Word. I don't think that's the problem. - Elliott

Exactly how did you add the shortcut to the tool menu?
Any other add-in(s) running?
