I am trying to find out how to scan a form into OCR. Do anybody know how?

You will need to use an OCR program that works on forms... not all do.
A Google search on "OCR forms" turns up 636000 hits.... one of them
should give you more information. I don't personally use forms
processing OCR software, but I've heard that ABBYY Finereader works
well.

If you want ocr, you want the text to be editable, i.e., readable in Word.
Forms usually involve graphics, even if it's just boxes & lines. To get a
good scan of that, usually takes an image scan, not ocr. Assuming you really
need ocr, the 2 best programs are Omnipage and Abbyy, both at $200+.
