Fieldcodes/Bookmarks/Macros

I am trying to create a document in word whereas when the document opens,
prompt boxes will appear, asking the user a series of questions (such as,
"Enter your address." Once the user answers the questions, the document will
be filled in with their response. I once created a document in a word
template like this and I can't remember how to do it. I know that I used a
bookmark, then the ref field code to create my question (I am not sure if the
bookmark or the fieldcode goes first) but once that is done, how do I use
macros to automate the document. By automate, I mean, the questions will be
asked as soon as the document open. The only thing I remember is that the
document had an AutoNew button or something like that...help I was trying to
use microsoft support but I can't figure it out. Thanks for your help.
