Find a Record on a Form (based on a query)

Good Morning,

I have a form that was built based on a query. I would
like to add a command button to find records based on two
fields on this form. I added a find button but that does
not give me the option to enter the two fields that I want
the criteria to search for. I would appreciate any help.
