Form for Engineering Standards

Is there a form already created that could be usable for Engineering
Standards? It would need a table at the top with text boxes and borders
around the page.

You probably will have to create this yourself.

Thanks, but I worded the question incorrectly. I know what the form needs to
look like, but am searching for a template that already exists. Our company
is considering purchasing Publisher and our primary use would be managing the
Engineering Standards. If there is not a template in already in existance,
it may not be cost effective to use Publisher for this application. MS Word
would be our other option.

I sincerely doubt that it would be so difficult to create a template for
this that it would push the project over budget.
