Form Results

I would like the form results that are posted to my email
to list the topics by name (as with a confirmation page)
rather than by field name:

(Email Results i.e.
-------------------
First_Name: July
Last_Name: Summer
Own_Car: Yes
--------------------
I would like the results to arrive like the confirmation
page results with the actual question and then the answer:
--------------------
First Name: July
Last Name: Summer
Do you own a car?: Yes
--------------------
....or, if that is not possible, can you have multiple
confirmation pages (one displayed, one hidden) that can be
accessed and printed out?

The form field names can not contain spaces
You can rename to your field names to say
First_Name: July
Last_Name: Summer
Do_you_own_a_car?: Yes

No you can not have 2 confirmation pages
