form submission is requiring password; how do I remove this

How do I remove from a form the funcion of requiring the submitter to give a
username and password? I have had this happen now with two websites that i
have up in the Internet. I cannot find any html coding requiring this of the
form submitter.

If you have the forms set to write to a file that is not in the _private folder, users will get
prompted for login access. The form can not be in the _private folder.

If the file is in the _private folder, then contact you web host and ask them to run a server health
check on each account / web site.

Hi,

I am having this exact problem, however it only happens for about 5%
of users. I have the form set to write to a file in the _private
folder as well as email the submission. For most users it works fine,
but a couple of people have said that when they press submit they are
asked for a username and password. Its not browser specific either,
had one of them test it in IE7 and Firefox and both exhibited the same
problem of asking for U and P. Pressing Cancel on U and P dialog
results in a blank white page...

Any ideas why this is happening to only a very few people? I suspect
a problem at the users end or a setting but don't know for sure...
Nor do I know how to resolve it.
