format of response to a form

If anyone can help, it will be GREATLY appreciated!

My form on website gives me a response in this format:
CHECK_BOX_3: yes,yes,yes, (etc)
depending on the number of boxes checked (their are 10 check boxes in 3
subjects)

my problem is I don't know which boxes are checked ! The form response does
not send the content of the text box referring to the check.
Any help?

Did you give them a value? If you didn't name each box, no way will the
form know what to send to you except the value.
