FP2002 Form Printer Option

I am the webmaster for a nonprofit org and have created a
membership form that is sent out to an email but prior to
an individual sending the completed form I would like to
set the option for the prospective member to print out
this form, in case he/she would like to mail (postal) or
hand carry it to the organization.

Does anyone know how to set a FP2002 Form with a printer
option button?
