help on forms

hey everyone,
i'm now starting to use forms and i need some pointers.
what i'm trying to create is a template that as a couple of fields and
dropdowns, then i want to come up with a way to:
1. copy the value that a text field is inputed with (fill in text field) to
another point in my template;
2. i have a drop down menu that either assumes numeric values (from 0 to
10). what i need is: if that field is greater than 0 then i want my template
to insert aditional lines for user imput information. it's sort of number of
relatives living with you. if the guy filling in the info inputs 1, then a
couple extra lines should appear (Name, Age, Ph number)

if you find troublesome answering it all, just point out a few websites or so.
