how do I add chapter numbers to an index using field switches?

BACKGROUND: I have set up a manual TOC and INDEX using {RD} fields instead of
creating a master document. In each sub-document, I have identified the
chapter heading with a {SEQ chapter} field. In the footer of each document, I
have used {SEQ chapter \c} - {PAGE} to get a result of 1-1. In the TOC, I
have used {TOC \s chapter \d "-" \t } to get a result of 1-1 in the TOC.
However, when I use {INDEX \s chapter} after marking my index entries,
chapter numbers do not appear.

MY QUESTION IS: What am I doing wrong? I have also tried using \b and
marking the index entries with bookmarks instead, but still could not get the
chapter numbers to print.

An additional complication is that I have appendices that are numbered
alphabetically that have roman numerals as page numbers. Is there any way to
get this type of numbering into the index?
