how do I create a check box to fill a value in a table

I am creating a questionnaire in Word where I want a user to check 1 of 5
boxes, in each of 3 sections, then depending if they check box 1, 2, 3, 4 or
5, the relative value would fill into a table whereby I can chart the results
for each section.

Example:
Section 1 - check level 3
Section 2 - check level 5
Section 3 - check level 1
Would generate a bar chart with 3 bars at 3 different values.
