How do I create a template that has a set of fixed layouts?

I want to control the layouts that Users can select when using my template.
I'd like to remove some of those available by Microsoft when you select a
slide and add my own with fixed text boxes, picture positions, tables etc.

You can neither restrict nor increase the number of layouts available.
Maybe you could with a huge amount of VBA - but be sure, the users will
find a way around it.

What you can do is create sample slides and save them with your
presentation. So that the user copies one of these sample slides instead of
inserting a blank new slide. And: Educate them! If someone understands the
reason why they should not use a certain layout, they will be much more
unlikely to use it.
