How do I create a toolbar in VB.NET

I've looked at SDK for 2003 already, but it discussed add=ins.

I would like to create a .NET toolbar in Visio, and call my app from that
toolbar. Does anyone have a blank project or tutorial that shows how to do
this?

The Flowchart sample in the Visio SDK contains CustomCommandBar class to
show you how
