How do I get the cash back offer from microsoft?

I've just purchased MS Office which is currently advertised with a cash back
offer. The guys at the store said I probably had to apply for it on this
website (but they weren't sure), and I can't find anything about it here,
also there is no information on the box about where to apply. Any suggestions?
It's Microsoft Student and Teacher Edition 2003, and I purchased it
yesterday 19/01/05. Nothing is mentioned about it during the install process
either.
Please help.

I probably should add that I live in Australia, and I can't find a reference
to Australia and the rebate on the address that you gave me. Only the US and
the UK. Am I missing something?

seems to no longer exist, the msg:
