How do I get the drop down feature to work in Word 2003?

I am creating a form and trying to use the drop-down form field and check box
form field in the form. I'm able to add my selections in the drop-down dialog
box, but when I go to choose in the form--can't. Have have a check in the
Fill-in Enabled box.

I can not get the check boxes to work in the form either.
