How do I print an Outlook custom form WYSIWYG?

I've created a custom form based on the mail message
template. Some users plan to print the form so they can
write answers in by hand, then key them in electronically
afterward. How can I print the form itself? When I
select File->Print, the result is just a list of all
fields on the form and their values.

See http://www.slipstick.com/dev/customprint.htm for various solutions to the limitation that Outlook forms don't support WYSIWYG printing.

FYI, there is a separate newsgroup for Outlook forms issues -- microsoft.public.outlook.program_forms.
