How do I transfer a form created in word into powerpoint?

i created a form as a word document but when i try to copy and paste it into
a powerpoint slide it is a) too big and b) split up into separate components
so impossible to fit onto the slide neatly aithout changing it's appearance.

A couple of ideas:

1. Scale down the Word window to see the whole form and do a screen capture
(ALT+PRINTSCREEN), paste it into a graphics program of choice, crop out
everything except the actual form, save it as a JPG or PNG, and do an
Insert/Picture/From file into PowerPoint.

2. Put a few bulleted items associated with the form on your PowerPoint
slide to explain the form. Put an Action Button below the text that
hyperlinks to the actual Word form (opens Word). When done viewing your
form, clicking the "X" at the top right will close out and send you back to
your slideshow.

If you need help on either of these, holler back.
