how does one print a label to the 5164 label in vertical format?

I am trying to find a template where I can compose a label for a product
using the 5164 labels but vertical.

I've done it before but don't remember how and suspect there is a much
easier way than what I did before anyway.

You could just create your label and rotate the contents 90 degrees.

Or you can setup your labels manually. Page setup, landscape, Arrange, Side
margins, .5, top & bottom, .16.
Grid guides, three columns, zero spacing, 2 rows, .19 spacing.
