How to create a form?

How do I create an interactive response/form with multiple
fields in an e-mail message (for recipients to fill out
and send/reply back)?

See http://www.outlookcode.com/d/forms.htm for basics but ... unless senders
and recipients are all in the same Exchange organization, an Outlook form
isn't a viable solution.
