how to setup a shopping cart and order form

Can anyone help me with the html code for directing the add to cart code to
an order form to complete the circle of an automated shopping cart. I have
added check boxes that say for the item to be added to the cart but the
connecting code to add that to the order form is a mystery that eludes me. I
would appreciate help.
