How To Start Forms Programming?

Where is a good place to go and/or a good book to get to get the basics of
programming forms in Outlook. I have Outlook 2K on XP and 98 clients. I
need to be able to create, what I have been told is, custom
receiving/sending forms in Outlook that are the default for e-mail. I need
to add some controls (checkboxes, etc.) to the form and make some decisions
based upon those controls. Then I need to update our Access97 (I know ...)
database through VBA (I assume). I'm sure the code won't be too difficult,
just looking for a good starting place so I don't make bad design decisions.
