how to write a reprimand?

Could someone help me to write a reprimand
I am not good at this and have to do one on an employee

Cover his/her GOOD points.
Cover his/her NOTSOGOOD points.
"However," deliver the reprimand.

Deliver the reprimand PRIVATELY,
VERBALLY and
FACE TO FACE.
You will gain much respect in the long run,
and improve your own self-esteem to boot!!

Acknowledge his/her being upset.

Give him/her the written copy on leaving to
ensure that there is a concrete statement of
his/her GOOD, INDIFFERENT and BAD point(s).
