I need a template form to file a mechanis lien

There is none since liens are subject to state and local laws.
You need to talk to an attorney regarding this matter.

Usually local municipalities have these forms available for free, it's
standard stuff but vary from city to city....just pick one up and scan
it...if you need a bunch, turn it into a fillable pdf form.

I tought each State would have a blank form for this purpose and I found lots
of helpfull information at library.findlaw.com ,lots of legalforms to
download, information on statutes and codes for each state, just in case
someone needs anything related to this subject that's the site to go to.
