Importing Contacts with Different Form

I was able to set a new form to use when creating a new contact. When I
import contacts from a CSV file they take the MS default form instead of the
one I created.

Is there a way to do this?

Outlook doesn't support importing to a custom form. You'd have to write custom code or use a third-party application. See http://www.outlookcode.com/d/customimport.htm.
