indexing problems

I am trying to index my Family History book. All the index names show up but
one group. I have about 6 generations that I need to show in the book. I
have indexed as follows
{ XE "Carolan:phillip & Mary (Sexton):John & Johanna:phillip & Effie:Joseph,
Joe & Dorothy:Donald & Joan:Rodney:Adam" \b}
The last name Adam does not show in my index. How can I make this
generation show up.
Is there anything to do to make more generation (names show up)

I'm getting the same result here. There doesn't seem to be any way to
specify the number of levels to include in an index, and the index styles go
up to nine (just like headings, TOC, etc.), so I'm at a loss to understand
or explain why this entry would be truncated. There are (very large) limits
to the number of fields in a document, the number of switches in a field,
and so on, but AFAIK there is no limit to the length of a field, and if
there were it would probably be at least 255 characters, and you're nowhere
near that.
